Antibiotic prophylaxis and treatment of surgical site infection in hip arthroplasty

Abstract

In orthopedics, is great the risk of surgical site infection (SSI) in complex surgeries like hip arthroplasty. The onset of infection leads to longer hospitalization, increased hospital costs and morbimortality. Appropriate use of antibiotic prophylaxis is one of the most important strategies to decrease the incidence of SSI. This study investigated the antibiotic prophylaxis and treatment of SSI in all type interventions hip arthroplasty performed in a federal hospital in Rio de Janeiro between March 1 and August 31, 2010. We realized a descriptive study about the drug use. All collected information was recorded in a spreadsheet developed for the study itself. Data were analyzed for suitability to the protocol and recommendations of the Infection Control Committee (ICC) institutional. The incidence of SSI identified of the 278 procedures investigated was 7.2%. The standardization of schedules of drug administration by the computerized system committed to the proper use of antibiotics as prophylaxis. It was observed that during the treatment of infection, 85% of prescribers suited the ICC recommendations. The rate of infection found and adherence to ICC recommendations demonstrate the presence positive ICC in the institution. The introduction of computerized systems to support the distribution of medicines in hospitals should be performed and make sure that the tool does not interfere with the rational use.
