Implementation of a contact phone service after hospital discharge: experience report

Objectives: to describe the results of Contact Telephone Service Post Discharge (SCTPA) hospital a teaching hospital. Methods: A retrospective, descriptive study conducted by analysis of CTSPH records held December 2013 to January 2015. The hospital where the study was conducted is divided into lines of care and there are multidisciplinary teams where professionals work, which were directed to care of patients with cerebrovascular accident (CVA), frail elderly, hip fractures, palliative care, and patient care with diabetic foot commitment. The results were described by staff in order to identify pharmacotherapeutic problems related to each profile. Results: we performed 111 telephone contacts, belonging to 108 patients (three readmissions). The average of the mean hospital stay was 19.7 days. The average number of medications used in the period before hospitalization was 6.8. Among the main problems identified pharmacotherapeutic, access was the most common, ranging from 17.5 to 37.5% between the teams. Not knowing the importance of using medicines correctly was the second most frequent in hip fractures and frail elderly. In diabetic foot impairment was the use of different prescribed drug (15%); AVC in the irregular use (18.5%) and in palliative care, not knowing that the drugs served (25%). Conclusion: access to medicines was the most prominent issue in all teams. These results can help to develop strategies to ensure greater access, effectiveness, safety, and adherence to treatment.
