Pharmacotherapeutic follow-up of asthmatic patients in ambulatory care service of Ceará: clinical outcomes

Objectives: To delineate the pharmacoepidemiologic profile of asthmatic patients, as well as to identify drug related problems (DRP) during the pharmacotherapeutic follow-up (PTF) in a specific health care service.
Methods: The study is a descriptive and prospective study in the area of Pharmaceutical Care (PC) involving asthmatic patients (n=30) under PTF of one year length, according to PWDT (Pharmacist work up drug therapy) method. The interventions carried on included direct education actions, teaching how to use properly the inhalers devices and patient and prescriber counseling. As to asses the primary outcome of this study ‘asthma symptoms control influenced by the pharmaceutical counseling’, each patient received a Diary Register of Asthma Symptoms formulary, which included: absence or presence of symptoms, amount of times of relief short action bronchodilator (SAB) use and expiratory flow peak measure. All the data were analyzed on SPSS program version 11.0, performing the Student t test for comparisons, considering a level of statistic significance of 5%.
Results: The patients of the study averaged 1.8 DRP. The inadequated pharmacotherapy generated 64 DRP, being distinguished those problems related to treatment compliance (31,2%; n=20), which were caused, mainly, by the misunderstanding of the information provided by the physicians. The main DRP were related to the efficacy and safety of the medicines, occurring, respectively, due to inappropriate use of the inhalers and mistaken medicines administration and/or dosage. After the interventions, the nocturnal expiratory flow peak measures increased significantly, not being necessary the use of SAB.
Conclusion: Our results suggest that the provided service of PC seems to be able to optimize the use of medicines, to diminish the symptoms of the illness and to improve the state of health of the asthmatic patients.
