Pharmacoeconomic analysis of cost minimization for protocol antibiotic therapy for community-acquired pneumonia in a hospital in Rio de Janeiro /RJ

The study aims to examine the clinical protocol for community-acquired pneumonia of a state hospital in Rio de Janeiro/RJ and tries to propose changes to it according to pharmacoeconomic evidences. A systematic literature search was conducted in the Base Virtual de Saúde (BVS), using the following associated descriptors: Pneumonia AND Economia Farmacêutica and Pneumonia AND Quimioterapia AND Protocolo. Articles which were far from the Latin America reality or far from the studied subject were not taken into consideration. For the cost analysis, ABCFarma magazine and Consulta Remédios website were consulted. After careful analysis of the selected articles and the hospital’s protocol, it was observed the best combination would be a beta-lactam with a macrolide. The use of a fluoroquinolone alone was considered in the case of allergy to beta-lactams. In accordance with the hospital’s standardization, a new therapeutic protocol that was consistent with what was discussed in the literature search was proposed. The clinical analysis confirmed the use of ampicillin as first choice treatment with azithromycin, leaving cefuroxime as 2nd choice. The amoxicillin/clavulanate and ceftriaxone were placed for 3rd and 4th choice for pharmacoeconomic reasons.
