Analysis of antimicrobial prescriptions for patients with acute renal injury

Abstract

Aim: The aim of this study is to identify anti-infective dosage adjustments in prescriptions to patients in a teaching hospital who presented alterations in creatinine clearance measurement tests. Methods: all creatinine clearance tests performed from January 2009 to March 2012 in patients in a teaching hospital were identified. Antimicrobial prescriptions for these patients issued within 48 hours after the release of their test results were also identified. Depending on creatinine clearance values, the prescribed anti-infective dosages were checked against scientific literature in order to identify the need of adjustments. Results: 50 patients who underwent the tests in the period extending from January 2009 to March 2012 were identified. These patients were given 53 prescriptions 48 hours after being tested. Four prescriptions had incorrect dosage adjustments as regards three active principles. Conclusions: The study shows that adjustments are actually made at the institution, although they were not performed in 100% of the patients in this survey. Divergent information as to dosage adjustment has been found in the literature.
