Analysis of the aspect of antimicrobials prescriptions at the medical clinic of a public hospital of Pará

In the hospital care practice, the antimicrobials are among the most prescribed drugs and their irrational use implies bacterial resistance, increased costs and decreased quality of life of patients. Retrospective study was conducted (2010/ January to June of 2010) in the medical records of patients between 19 and 64 years old, hospitalized at the Medical Clinic of the Hospital Regional Dr. Abelardo Santos in the city of Belém, State of Pará. The sample was 63 records, showed 100% empirical therapy in relation to specific therapy. Respiratory infections, urinary and gastrointestinal were the most frequent and the median of the hospital stay was 7 days. The classes of beta-lactams and quinolones were the most prescribed, and as representatives of classes of the ceftriaxone (52 prescriptions) and ciprofloxacin (21 prescriptions), respectively. The time of therapy with antibiotic ranged between 1 and 17 days. Was a big variety of associations of antibiotics, even within the same pharmacological class of drugs. For 68.3% of patients, treatment was with polytherapy (2 to 5 antimicrobials). The data show the importance of creating mechanisms to control the prescribing and dispensing of antimicrobials, restrictive actions for the use of such medicines and educational actions for the health team. The rational use of antimicrobials depends on changes in prescription practices and involves different players, including the Hospital Infection Control Service, the Hospital Pharmacy Service, the Laboratory of Microbiology and Direction of the Hospital which should support decisions made in order to obtain health assistance quality.
