Patients’ perceptions in a teaching hospital about pharmacotherapy and pharmaceutical care in hospital discharge

The high costs of hospitalizations, together with the advances in health sciences, have contributed significantly to the reduction of hospital stay, leading patients to early discharge. Studies that evaluate the knowledge of the patient about the medication concluded that it is poor the level of information. In this context, clinical pharmacists can contribute to achieving positive clinical outcomes.
Objective: To identify the perceptions of patients admitted to a teaching hospital about their pharmacotherapy and pharmacist counseling at the time of hospital discharge.
Method: Prospective, descriptive, exploratory, held at the HU-UFJF between October and November / 2011. Questionnaires were applied to patients at discharge.
Results: We evaluated 30 patients with an average of 12.4 days hospital stay: 80% reported ignore the role of the pharmacist in hospitals; 50% knew the purpose of the use of all medicines; 50% stated that their knowledge of the drugs were "complete"; 90% said they received information about the use of medicines; 77% did not receive information about drug adverse reactions and 93% would accept have a conversation about medication with the pharmacist before leaving the hospital.
Conclusions: The rational use of medicines cannot be occurring given that the pharmacist is not inserted in the discharge process and that there are possible gaps in knowledge presented by the patients. We emphasize the uniqueness of this locus of human resources training, highlighting the need to insert pharmacist at hospital discharge and potential of education activities.
