Abstract

The paper notes the frequency of occurrence of dispensing errors related to drugs Look Alike / Sound Alike, intended for patients Care Unit for Children's and Adolescent’s Health at the University Hospital Onofre Lopes, before and after the application of Tall Man lettering tool. This analysis aimed to assess the possibility of lowering Medication dispensing errors Look Alike / Sound Alike after the application of this tool. The search was performed using an experimental study in two stages through the inductive method, or empirical. The first phase without use of Tall Man lettering and second phase using the tool. Data were analyzed using SPSS software, where compared error rates between 2 steps with the chi-square test (p <0.05). It showed a greater amount of error after applying the tool when compared to the level without the tool. The drugs of the cardiovascular system (Amlodipine, Captopril, Carvedilol and Hydrochlorothiazide) were the most frequent (21.8%) in the dispensing error involving LASA drugs. Obtained result p = 0.33, i.e., the differences in the two phases of the study were not statistically significant. In conclusion, in this study, the tool, in isolation, was not helpful to ensure improved patient safety.
