Abstract

Objective: To develop and make available a quick reference guide on Y-site incompatibilities between the drugs most commonly used in critical care.

Method: An initial survey was conducted of the most commonly intravenously administered drugs in the Intensive Care Units (ICUs), including antimicrobials, and then those identified were cross-checked for y-site compatibility using a database. A table was then designed containing this information.

Results: 88 drugs were included. The instrument developed was made available and integrated into team training.

Conclusion: This study has provided an information source capable of optimizing safe drug administration, highlighting the benefits of including a clinical pharmacist in multidisciplinary care teams.
