Supply problems with antiretroviral drugs: fractioning at haart initiation

Objective: This study aimed to describe the problem of fragmentation in the antiretroviral drugs dispensing, analyze its consequences and determine the adherence percentage during the first year of drug therapy in HIV näive patients.
Method: The study was conducted from September 2007 to February 2010 in the Pharmaceutical Care Department at the Hospital Nacional de Clínicas “Dr. Pedro Vella” in Córdoba (Argentina).
It is an analytical-descriptive (quantitative) observational study. The population included was divided into two groups, depending on whether they initiated treatment receiving drugs for a complete month (full treatment), or for fewer days (fractionated treatment).
Variables to be measured: Adherence rate, number of missing doses (in theory), amount of opportunities patients attended hospital looking for the medication, and number of occasions people should leave without the therapy.
Data were obtained from dispensing records and pharmacotherapy follow-up files.
Results: According to the data obtained, adherence would not have been affected by fractionated dispensing. Adherence achieved by the group starting complete HAART was 75.84%; and by those receiving drugs for a few days was 70,16%.
Conclusions: None of the groups under study reached a 95% adherence required during the first year of treatment. Fractionation of treatments would not have been a cause for which this goal was not achieved.
