The sample consisted of all medical records of patients followed by pharmaceutical resident in Multidisciplinary Clinic of Geriatrics and Gerontology at a university hospital in southern Brazil.</p><p>Results: We identify 86 drug-related problems and possible negative outcomes, which required pharmaceutical interventions, among 27 elderly. Participants who accepted the interventions (74% of the total participants) showed a statistical significant improvement/resolution of problems in 60% of cases (p=0.008).</p><p>Conclusions: The pharmacotherapy implies negative outcomes for the health of patients. The drug-related problems identified by pharmaceutical was high, highlighting the insecurity afforded by the use of inappropriate drugs, drug interactions and adverse drug reactions. Pharmaceutical care contributed significantly to the rational use of drugs among the elderly assisted in the ambulatory.</p> ER -
