The mean of age of patients was 77 years, 224 were female , 131 were male. Observed an increase in the proportion the number of prescriptions analyzed and number of interventions with the time offering the service. Constipation and rash were the most common adverse reactions. The most drug interactions afforded risks involving the central nervous and cardiovascular system.</div><div>Conclusions:</div><div>The organization of the pharmacotherapy follow-up service and the integration of</div><div>multiprofessional resident pharmacists in care of the elderly led to the implementation of clinical care actions of the pharmacist in elderly care in a emergency urgency hospital , and contributed to the safe use of medicines.&nbsp;</div> ER -
