Preparation and administration of medications: errors made by the nursing staff

Objectives: To identify the main errors in the preparation and administration of drugs made by the nursing staff of a public hospital in the state of Piaui.
Methods: This exploratory-descriptive cross-sectional study with a quantitative approach, developed in a Public Hospital State secondary school located in the city of Picos -PI, which serves patients from 39 counties of Macro-region of the Valley of Guaribas.
Results: The mistakes made in the preparation of most reported drugs were: preparation of various medicines schedules and different patients in the same tray (48,9%), occurrence of side conversations during the preparation (44,4%), failures related to biosafety standards (40,4%), wrong time (37,8%) and wrong medication dilution (35.6%). With regard to failures related to drug administration, the most frequent were: biosafety standards (55,6%), wrong administration time (46,7%), dose omission (40,0%) and speed of incorrect infusion (37,8%) .
Conclusions: Medication errors have multifactorial causes. Thus, should be analyzed in a multidisciplinary way, so that preventive measures are implemented. The proposal is that changes occur, and that these should start with small obstacles, including the entire medication process and identifying the changes that are likely to result in improvement.
